To refer a child for trauma therapy, call our office at 727-845-8080 or utilize this Trauma Treatment Referral Form. Completion of this referral does not guarantee acceptance into services. A Victim Advocate will contact referral source to determine eligibility. Please email referral form to Tammie.Gernand@flhealth.gov.
