The Nurse Home Visitor will provide virtual and/or in home visitation in West Pasco County to substance-affected infants and their families who need medical/developmental consultation and treatment by a Licensed Practical Nurse (LPN). He/she works under the guidance and direct supervision of the Healthy Families Supervisor.
ESSENTIAL FUNCTIONS: Duties of the Nurse Home Visitor (NHV) include but are not limited to:
- Performs regular home visits with families as determined by model guidelines and contract requirements, maintains caseload limits, and meets the expected home visit completion rate each month.
- Establishes a trusting relationship with families assigned to caseload by initiating and maintaining regular client contact and strives to keep families engaged in the program. Visits should focus on linking pregnant women with prenatal care, promoting strong parent-child attachment and coaching parents on learning activities that improve their Substance-affected Infant’s development.
- Create or modify a Plan of Safe Care (PSC) for all Substance-affected Infants and their families who consent to a (PSC). Participates in staffing’s with partner agencies as necessary.
- Seeks cooperation of all household members and identifies and addresses any needs of individuals living in the same home as the substance-affected infant.
- Identifies and assists families in accessing needed resources and services.
- Ensures that family is linked to a medical provider; provides information on the importance of timely immunization and well-childcare; and document dates of immunizations and well-child check-ups.
- Along with the Program Manager, collaborates with community partners including hospitals, MAT clinics and other treatment providers, OB/GYNs, midwives, pediatricians, child protective investigators, county health departments, or any community or state entity providing referral services.
- Collaborates with other medical professionals in the community through Pasco Kids First partner agreements. Discusses these collaborations during supervision. Asks for assistance and/or guidance whenever necessary and as needed.
- Helps families establish a Family Goal Plan with goals, objectives, and activities and follows through with implementation. Provides follow up to identified concerns and monitor family progress.
- Administers developmental assessment tools at established intervals, and other established assessments according to policy (including but not limited to: ASQ-3, ASQ-SE, Healthy Families Parenting Inventory, Home Safety checklist, Edinburgh).
- Promotes and models effective parent-child interaction through teachings and activities that enhance and strengthen bonding and parenting skills.
- Utilizes trauma-informed approaches and motivational interviewing techniques with families.
- Prepares home visit notes and other documentation in a timely manner and comply with agency and program requirements, policies and procedures relative to the proper establishment, handling and storing of client records.
- Maintains confidentiality of participant information and respect diverse cultural differences of families within communities.
- Receives supervision related to the families on their caseload at least weekly, with an emphasis on review of case management and other relevant activities of all families on caseload as well as ensuring collaboration with FSWs on addressing needs and concerns, whenever appropriate. Receives Clinical and Reflective Supervision – Open to reflective practice (i.e. has capacity for introspection, communicates awareness of self in relation to others, recognizes value of supervision, etc.)
- Participates in case planning and supervision, along with the FSWs, for families on their joint caseload, whenever appropriate.
- Manages time and resources effectively through organization, prioritization of work, and positive problem-solving skills.
- Demonstrates a strong work ethic and participates in the organization’s performance and quality improvement efforts.
- Travels as required within the service area for hospital visits, home visits, educational programs or meetings; mileage will need to be correctly documented, compiled and submitted monthly for reimbursement.
- Provides support to all Family Support Workers when working with families where substance abuse has been identified as an area of concern and conducts joint visits with FSWs throughout a family’s participation in the program, whenever appropriate for the family.
- Completes required Healthy Families trainings and attends conferences/workshops, in-house trainings, staff meetings, and other staff development activities as required and in a timely manner. Assists with in-house staff trainings related to medical/developmental consultation when necessary, and as instructed by supervisor or Program Manager.
- Submits all required data in the specified timeframe as determined by the program supervisor.
- Participation in the Pasco and Hernando County Substance Exposed Newborn Taskforce meetings and the Pasco County Alliance for Substance Addiction Prevention (ASAP) meetings are required.
- Performs other related duties as assigned.

MINIMUM QUALIFICATION REQUIREMENTS:
Education & Experience:
- Must be a Florida Licensed Practical Nurse in good standing; bilingual preferred, not required
- Must have at least 2-years of experience in maternal/child health, public health, or mental behavioral nursing
- Must be able to work flexible hours to meet the clients’ needs
- Experience with home visiting preferred
- Experience in working with or providing services to children and families; knowledge of infant child development preferred
- Acceptance of individual differences and experience and willingness to work with the culturally diverse populations that are present among the program’s target population
- Open to reflective practice (i.e. has capacity for introspection, communicates awareness of self in relation to others, recognizes value of supervision, etc.)
Licenses & Certifications:
- Current State License as LPN
- Must possess and maintain a valid Florida driver’s license with no record of criminal driving offense of license suspension.
- Must possess means of providing job related transportation and show proof of required insurance.
- Must successfully complete and maintain FDLE clearance, Federal background and state criminal background check.

Annual Training Requirements: The following training topics are required annually: Diversity/Cultural Competency, Child Abuse and Neglect Mandated Reporter Statutes and Methods, Department of Children and Families HIPAA and Security Awareness, and ADA compliance.
