Family Assessment Worker
This position provides assessments for potential participants in the Healthy Families Pasco / Hernando program.
Duties and Responsibilities:
Assesses expectant families and families of newborns in the identified zip codes using the HFF Assessment Tool after receiving HFF Assessment Core Training.
Provides a minimum of 80% of assessments prenatally or within two weeks of birth.
Provides 100% of assessments within 90 days of birth.
Obtains written consent from the family before assessment is conducted.
Works with area hospitals to ensure that mothers residing in the targeted zip codes who give birth are provided with an opportunity for assessment within 90 days of birth.
Develops relationships with and works with physician offices and agencies that serve prenatal moms such as Healthy Start, SOLVE, Health Resource Alliance, IPO, PCHD, Pregnancy Care Center, Catholic Charities, DCF Economic Services, schools, Baby Love, A New Generation, The Life Center, etc. to ensure that prenatal moms are referred to Healthy Families Pasco / Hernando.
Assures that all eligibility and services are tracked and reported to the Program Manager.
Is available for weekly supervision by the Program Manager or Assistant Program Manager.
Offers referrals for community-based services to those families not eligible for or who refuse Healthy Families services.
Provides written assessments to Field Supervisor and provides input on assignment of family to RM.
Works a schedule of forty hours per week at times that are convenient to the families being assessed.
Attends team meeting and reports out on assessments.
Attends training and other meetings as required.
Maintains necessary paperwork on a current basis.
Performs other related duties as assigned by the Program Manager.
Knowledge, Skills and Abilities:
Knowledge of community based resources and basic social work principals.
Knowledge of home visiting practices.
Ability to engage families in a trusting relationship.
Ability to complete a comprehensive psychosocial assessment to determine families needs.
Ability to communicate effectively, verbally and in writing.
An Associate’s Degree in human services field. A high school diploma and completion in post high school training in a course of study that involves assessment skills plus five years of work experience in that area may be substituted. A Bachelor’s Degree is preferred.
Valid Driver’s license and access to reliable transportation.
